New Patient Registration
We are happy to accept new patients. Please take time to register when you move into the area (map opposite), preferably in advance of your first appointment.
We invite all newly registered patients to complete relevant paperwork, which will be followed by an appointment with a nurse. It is often very useful to keep a repeat slip from your previous surgery, as it can take a few weeks before we get your medical records.
In order to ensure we have all of the relevant information to register you as a new patient, please take a few moments to complete the forms below - thank you. Please note there are different forms for adult and child registrations.
Adults
GMS1 Registration Form - this is your registration form and must be completed.
Adult New Patient Questionnaire - this will help us to ensure we have all of your current medical information before we receive copies of your medical history from your previous GP.
Information Sharing Consent form - this is a very important form to ensure patients are aware of their options when considering who to share health information with.
New Patient Alcohol Screening - this is a short questionnaire designed to understand current alcohol intake.
Children
GMS1 Registration Form - to be completed by parent/guardian/carer of child to be registered.
Child New Patient Questionnaire - to be completed by parent/guardian/carer of child to be registered.
Information Sharing Consent Form - to be completed by parent/guardian/carer of child to be registered.
